POSITION SUMMARY
The primary purpose of your job position is to be accountable for the delivery of quality care to patients and families, according to the individualized needs of the patient. Responsible for the assessment, treatment, and care for patients of all ages including adult and geriatric, as applicable. Competent to access and analyze patient information and history with an understanding of the cognitive, physical, emotional/psychosocial, spiritual, and chronological maturation process. Accepts and promotes professional development as an integral part of nursing practice. Coordinate patient care activities with other members of the health care team in a collegial relationship. Directs and coordinates the care given by Certified Nursing Assistants, (C.N.A.’s), Licensed Practical Nurses (LPN’s), and ancillary personnel. Responsible for coordinating and facilitating organizational performance improvement. Responsible for the development of quality indicators, facilitates the collection and analysis of data, and serves as a resource person for quality improvement/patient safety activities across the continuum. Coordinates and facilitates facility awareness and understanding of the requirements of participation and any upcoming changes. Serves as a resource person for Villa staff in education on the requirements of participation, the QA/PI process, and time lines. Works with the Medical Staff and the Villa staff to assure understanding of what and how to document in the medical record to meet the documentation requirements thus validating that the requirements of participation are met. Assists in maintaining compliance with accrediting and regulatory requirements. Assists to ensure timely and accurate completion of the Federal and State assessment tools. Has an understanding of the MDS rules and regulations as described in the MDS User's Manual, including item coding, RAPS, CAA's Care Planning, electronic submission, scheduling, PPS including knowledge of MDS submission process, final validation reports, and an understanding of Medicare rules and regulations including coverage, benefit periods, certification, 30-day rule.
KNOWLEDGE, SKILLS & ABILITIES
Preferred, as a minimum, three (3) years Registered Nurse experience in an acute care hospital, long term care facility, or other related health care facility
PREFERRED QUALIFICATIONS
Must be able to write the English language in a legible and understandable manner. Must possess the ability to deal tactfully with personnel, residents, family members, visitors, government agencies/personnel and the general public. Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations, and guidelines that pertain to long term care and acute care.
WORKING CONDITIONS - ADA INFORMATION
Works primarily in skilled nursing facility setting. Requires considerable standing, mental effort, stooping, bending, lifting (in excess of fifty (50) lbs.), pushing objects (carts, beds, and wheelchairs). Likely to be exposed to blood and body fluids, including infectious diseases. May rarely require accompanying and assisting with patient transport to another facility. Uses medical, office equipment, and telecommunicates devices on a regular basis (e.g., I.V. pumps, fax, computer, and pagers). Physical functional capacity for healthcare workers as defined by the Saunders Method (very heavy).
