Department: DIABETES MANAGEMENT CLINIC
Schedule: Full Time, 8 HR Day Shift,

POSITION SUMMARY 

  • Provides patient care through assessment and implementation of the nursing process for all age groups cared for on the unit. 
  • Performs and assists with procedures and/or the needs of our staffed physicians and midlevel. 
  • Perform injections, vaccines and lab draws. 
  • Documents care provided. 
  • Complete all necessary paperwork. Example but not limited to-prior authorizations, prescription refills, DME, CMN, orders, referrals, procedure appointments. 
  • Calculate and administer insulin. 
  • Work with diabetic devices such as insulin pumps, meters and continuous glucose monitors. 
  • Coordinates patient’s plan of care with physicians and other disciplines. 
  • Maintains an atmosphere, which ensures South Georgia Medical Center’s mission, vision, values, code of conduct, goals, policies and standards are consistently supported and enforced. 
EDUCATION 

  • Current RN License in State of Georgia 
  • CPR required 
EXPERIENCE 

Three to Five (3-5) years nursing experience in related specialty or primary care area preferred 

KNOWLEDGE, SKILLS, AND ABILITIES 

  • Ability to work well individually and as a team leader or member. 
  • Demonstrated ability to assess and, adapt to new situations, think critically, manage priorities and utilize problem-solving techniques. 
  • Ability to read and interpret policies, standards, and think critically evaluating patient care issues. 
  • Strong clinical nursing skills in assessment, planning, intervention and evaluation. 
  • Ability to communicate clearly with, and care for diverse population of patients/family units of all ages, a variety of cultures, and/or persons occasionally exhibiting stressed behaviors. 
  • Ability to operate clinical equipment, computer, telecommunication devices and patient information systems. 
  • Ability to lead, motivate and develop others individually and as a team. 
  • Ability to adapt to new situations, set priorities and utilize problem-solving techniques. 
  • Strong nursing skills in assessment, planning, intervention and evaluation. 
  • Excellent critical thinking skills as well as verbal, written and behavioral communication. 
  • Ability to direct the development and implementation of policies, procedures, and patient care protocols. Demonstrate current knowledge of patient care especially regarding Standards of Care for the patient with diabetes and co-morbid conditions (hypertension, dyslipidemia, etc.). 
  • Strong ability to read, interpret and analyze regulations, standards, clinical issues, and outcomes. 
  • Ability to access learning/training needs of healthcare personnel. 
  • Flexible in work schedule, assignments, workloads, setting priorities and utilizing problem solving techniques. 
  • Utilize variety of office, educational, clinical and non-clinical equipment. 
  • Proficient with computers especially Microsoft Office – email, Word, PowerPoint, etc. 
  • Utilizes computer software/EMR/internet to review patient progress/patient goals/patient risks 
WORKING CONDITIONS - ADA INFORMATION 

Works primarily in hospital or clinic setting. Requires considerable standing, mental effort, stooping, bending, lifting (in excess of fifty (50) lbs.), pushing objects (carts, beds, and wheelchairs). Likely to be exposed to blood and body fluids, including infectious diseases. May rarely require accompanying and assisting with patient transport to another facility. Uses medical equipment, telecommunications devices and patient information systems on a regular basis. Physical functional capacity for healthcare workers as defined by the Saunders Method (very heavy).
