POSITION SUMMARY 

The Care Coordinator will be responsible for patient Intake, Case Management as well as Marketing of the Senior Behavioral Care Unit. The Care Coordinator will work directly with the unit manager and other licensed staff on the Unit along with the Medical Staff and patient and/or patient families to ensure timely, appropriate admission, treatment planning and DC Planning for the individual needs of the patient 

KNOWLEDGE, SKILLS & ABILITIES 

  • Must demonstrate attention to detail, experience in marketing and social media platforms. 
  • Must possess strong analytical skills in assessment, planning, and evaluation of resident placement for GPU unit.
  • Must have positive attitude with demonstrated ability to handle delicate situations and to communicate effectively with members of affiliates, physicians and other referral sources, patients/families, and community agencies/institutions.
  • Must possess ability to work non-judgmentally with persons of diverse cultural, religious, and socioeconomic backgrounds and lifestyles.
  • Must be self-motivated with strong organizational skills and demonstrated ability to utilize critical thinking techniques and problem-solving techniques with minimal direction and appropriate collaboration.
  • Ability to operate office equipment such as computers, telecommunications devices, etc. 
  • Must have current GA driver’s license and satisfactory driving record 
  • Must have current CPR and First Aid certification 
  • 1 year mental health experience preferred 
  • Active RN or LPN license in the State of Georgia. CPR certification required. CPI certified. 
WORKING CONDITIONS - ADA INFORMATION 

Provides 24/7 oversight for all referrals to GPU. Works primarily with medical staff and unit manager to provide quick assessment and verification of appropriate admissions to the unit. Visitation with outside referral agencies/businesses or affiliates within a community setting. Reach out in person to all ERs within a 50 mile radius weekly. Reach out weekly to all primary referral sources by text, phone, in person. Reach out bi-weekly to secondary referral sources by text, phone, in person. Must attend 2 community events a month. Perform additional duties as assigned. Mental effort to assess, plan and make decisions in complex situations. Ability to operate basic office machines such as telephone, fax, copies, and computer equipment.
